Job Description Job Description ESSENTIAL FUNCTIONS
Assessment and coordination of provider schedules to organize patient flow
Input patient care orders after provider has noted them in patients progress note
Work collaboratively with patients to reduce barriers to accessing mental health services
Collaborate with Behavioral Health Navigator to make referrals as authorized by physicians to managed care companies on behalf of InterMed patients
Build a rapport with mental health facilities and practitioners to ensure collaboration and information exchange between InterMed behavioral health and community resources
Contact patients to confirm they have made contact with the specialists’ office
Answer incoming calls from internal customers, patients, and specialist offices
Document all clinical data correctly and appropriately in patient’s EHR
Communicate patient information after consultation with provider
Preload patient records prior to routine visit per provider protocol and oversight
Refill patient medications according to protocol per provider approval and oversight
Oversight of clinical environment with attention to safety and infection control
Provide follow up with patient after hospital and /or skilled facility discharge
Complete PMP checks and manage forms such as PHQs, Vanderbilt assessments with parents and schools, and GAD 7.
Have an understanding of workflows and processes for Behavioral Health intakes and assessments
Properly triage for depression, crisis, and suicide issues
Collaborate with the Behavioral Health team to review all referrals for appropriateness.
Places follow up calls for medications and checks in with patients and parents as requested by the provider
Provide patient nursing care; facilitate patient education
Provide nurse care management for patients enrolled in Collaborative Care
Coordinate and support individual, group and population based health maintenance and promotion
Coordinate patient care management with in house specialties and resources; community resources; home health and other health care agencies; hospital care
Manage telephone encounters utilizing triage and telehealth principles
Excellent professional judgment, phone skills and decision making ability
Work collaboratively with all team members
Demonstrate initiative, responsibility, flexibility and decision making
Ability and willingness to provide excellent customer service to patients, InterMed colleagues and vendors
Maintain confidentiality and compliance in all patient care activities
Special projects as assigned by lead, coordinator or manager
JOB REQUIREMENTS
Graduate from an MA program
Active Certified MA
Experience working in a mental health setting preferred
Professional appearance and demeanor
Proficient in Windows applications, electronic medical record preferred
SCHEDULE
Monday through Friday, 40 hours per week with 5, 8-hour shifts
