Care Coordinator

The Care Coordinator plays a vital role in providing essential support services to individuals, including assessments, service plan development, case management, skills training, group facilitation, linkage to resources, and crisis intervention. The Bilingual Coordinator is responsible for managing a caseload of Managed Care Plan members, ensuring regular contact as stipulated by the Program Contract(s).
Responsibilities:
Manage a caseload of approximately 40-60 Managed Care Plan (MCP) Members, delivering services in locations that are most accessible and within MCP guidelines.
Connect MCP Members to additional social services and supports as required, advocating on their behalf with healthcare professionals.
Implement motivational interviewing, trauma-informed care, and harm-reduction practices to support MCP Members effectively.
Collaborate closely with hospital staff on discharge plans for MCP Members.
Arrange transportation and schedule in-person visits with the care coordinator for MCP Members.
Prepare assessments, progress notes, and other clinical documents accurately and in a timely manner.
Conduct assessments, develop service plans, manage cases, provide psychosocial rehabilitation, offer crisis intervention, facilitate program transfers, and oversee discharges.
Develop and lead psychosocial rehabilitation and co-occurring skill groups for MCP Members.
Maintain a consumer caseload, ensuring productivity levels meet assigned targets each month.
Actively participate and contribute positively during clinical supervision.
Minimum Qualifications: High school diploma or equivalent (required).
A minimum of four years of direct care experience in a mental health-related field; OR two years of direct care experience in a mental health-related field, AND two years of education in a mental health-related subject area such as child development, social work, human behavior, rehabilitation, psychology, or alcohol and drug counseling.
Experience working with older adults is preferred.
Might work some weekends.
Bilingual in Spanish , required

Pay Details: $22.00 to $25.00 per hour
