**HOME CARE PACKAGE MANAGER**

Just Better Care Hawkesbury Nepean Blue Mountains are a leading provider of in-home community support, enhancing the lives of people in our community. We are currently seeking an experienced Package Manager to join our friendly team!
This role will require you to work within a diverse role, developing and managing relationships with new and existing CDC Home Care Package Consumers and Privately funded clients, ensuring care is provided in accordance with the relevant Professional and Industry Standards, appropriate Program Guidelines, and all legislative requirements.
The Package Manager performs an integral role, assisting current Package Managers in ensuring individualised services are delivered to customers across all programs and align with program guidelines. You will be responsible for assisting with the development, maintenance and review of individualised support plans in consultation with the customer, their family or guardians and the key health professionals.
**Core Responsibilities:**
* Undertake comprehensive customer assessments for Home Care Package clients, including those with complex health needs
* Assess, monitor, reassess and review the ongoing needs of the customers and work in conjunction with health professionals, where required, to ensure the health and wellness of all customers
* Ensure appropriate information is provided and explained to the customer at all times
* Where applicable, develop strategies, which will address the identified needs of the customer in consultation with their representative/s where appropriate, in accordance with funding guidelines
* Demonstrate a commitment to best practise, ensuring that all services are provided in a caring and respectful way in accordance with Just Better Cares policy and procedures, and legislative requirements
**Essential Requirements**
* Previous experience within a similar role in the aged, disability or community services sector
* Ability to work with clients to create and implement individualised Care Plans
* Ability to organise various services, trades and purchases for customers, in line with guidelines, whilst maintaining the customers budget
* Strong organisational skills and the ability to coordinate services according to relevant program guidelines
* Ability to develop and maintain links and supportive networks with specialist services
* Experience maintaining computerised customer information systems
* High level administrative and organisational skills
* High level of written and verbal communication skills
* Current and clear Criminal Record Check, NDIS Worker Check & Working with Children Check
* Current Drivers Licence
**Desirable:**
* Qualification relevant to the aged, disability or community services sector (preferred)
