Hyve Health is seeking a detail oriented and energetic Care Coordinator to join the Hyve team at our Chicago location! The ideal candidate is able to identify & anticipate both member and Clinical Team needs in order to execute exceptional care on all fronts.

Where you come in:

The Care Coordinator will be responsible for the overall organization and workflow of the Clinical Team. This role will focus on daily leadership and problem solving for the Clinical Team in order to treat members with high quality and personalized care. The ideal candidate for this position is a skilled multi-tasker, strong work ethic, and ability to thrive in a fluid work environment. This role will work closely with the Co-Founder & Pediatrician in Chief.


Create and maintain clinical workflows in order to preserve high quality member care

Assist in the daily coordination of clinical team processes

Develop, maintain and provide functional and administrative support

Own process of gathering, updating, and distributing all patient paperwork including previous records, school charts, vaccine logging, prescriptions, newborn screens, etc.

Direct member flow and facilitate assistance to maintain proper processes

Work directly with the Clinical Team to create and maintain patient care plans

Order medical supplies and maintain inventory under supervision of the Pediatrician in Chief

Aid in the implementation of systems that improves patient intake, flow, and member satisfaction. Provides feedback to ensure the efficient onboarding of members

Field all patient communication including answering phone calls and responding to messages

Communicate with clinical vendors including pharmacies, labs, etc.

Identifies and reports ways in which member care could be improved by investigating concerns, developing solutions and making recommendations

Ensure the Clinical team has access to necessary technology portals and communicates updates and changes in processes

Work collaboratively with multiple departments and clinical team to continuously improve programs, processes and organizational initiatives

Monitor and record quality control checks on specific medical equipment, office diagnostic tests following established protocol as directed

This Might Describe You

2+ years experience in medical administration a plus

Has a solid foundation in administrative medical knowledge a plus

Adaptive to changing environments and timelines with quick problem solving skills

Proactive, organized, detailed, and hands-on

Excellent communication, written, and interpersonal skills

Ability to juggle multiple projects and take initiative

Master multi-tasker

Passion for building and cultivating relationships within and outside of the organization
