**Description**
We are looking for a **Team Leader** to join our Foster Care program to be responsible for providing extensive case management and coordination for each client and family. The Case Manager must interface with outside providers and collaterals as well as work with families to provide ongoing child-specific skills training and problem solving within the home and help the foster parents to create a trauma-informed environment for the youth.
**Responsibilities** **Include:**
* Meet with client and family on a weekly basis in the foster home
* Carry a caseload of 4 clients
* Create individualized treatment plans and monitor progress towards goals
* Lead foster parent support groups
* Attend and/or facilitate meetings involving the client
* Complete accurate documentation in a timely fashion
* Manage a team of caseworkers as assigned
* Responsible for rotating after-hours on-call coverage
**Qualifications/Requirements**
* Bachelors Degree required. M.S.W., M.A., or M.S., or related
* Minimum of two (2) years experience in Human Services working with troubled adolescents and their families
* Ability to work with a team as well as independently
* Demonstrated ability to be innovative, creative, analytical and decisive in problem solving
* Demonstrated ability to work with other community agencies (i.e. School Dept., DCF, Juvenile Court, etc.)
* Bilingual English/Spanish speaking encouraged to apply.
