As a rehabilitation clinician, you will provide hands-on care, education, and training for patients as they recover from health conditions and injuries. In collaboration with the interdisciplinary care team, you will support your patients’ ability to remain safe at home. Position Summary: Develops implements and assesses physical therapy treatment programs for agency patients. Activities are conducted within the framework of Agency policies and procedures and are consistent with established professional standards.

Essential Functions :

Manages a caseload giving consideration to patient/family needs, travel efficiency and service priorities.

Plans, implements and evaluates individual physical therapy programs and diagnostic tests in conjunction with the prescription and/or the direction of a physician.

Other Duties:

Instructs patients and family members in supportive, preventive and therapeutic procedures.

Provides consultation to other Agency staff concerning the application of professional principles and procedures to problems in patient care.

Identify barriers to desired outcomes.

Evaluate equipment needs to increase functional level.

Accountable to Patient Care and QI Managers for attainment of standards of professional practice.

Completes records and reports necessary for patient service and administrative requirements and determined by the Agency.

Assumes responsibility for professional growth and maintains clinical competence.

Establishes and maintains effective working relationships with patients, families, peers, supervisors and community agencies.

Responsible for meeting performance standards including qualitative and quantitative standards, as stated by the Agency.

Identifies the need for services of other health care disciplines or agencies and initiates the appropriate referral.

Facilitates coordinated and comprehensive plans and goals for VNA patients.

Communicates regularly with Director of Rehabilitation or other health care personnel to develop coordinated plans for patient care.

Supervises Physical Therapy Assistant in carrying out the established plan of care and evaluates the effectiveness of the service.

Supervises Home Health Aide in carrying out the established plan of care and evaluates the effectiveness of the service.

Participates, as appropriate, in implementation of the Agency's Quality Improvements Program.

Participates, as appropriate, in Agency educational activities, including new employee orientation and professional student clinical training and Agency staff education programs.

Interprets Agency services and programs to individuals/families and personnel or other agencies.

Participates, as assigned, in community programs.

Carries out other activities relevant to the therapy service.

Complies with the Agency's Policies and Procedures. Contract therapist also complies with contractual arrangement.

Performs other related duties as requested.

Knowledge and Experience :

Possession of the PA Physical Therapy licensure.

Minimum of one year of direct care patient experience, preferably of a generalized nature.

Current driver's license and auto insurance.

Education: Applicants must have completed an educational program (Bachelors or Masters Degree) currently approved by the respective national professional organization.

Verbal Communication Advanced

Behaviors Required Team Player Team Player : Works well as a member of a group

Education Required Bachelors or better in Rehabilitation Services.

Masters or better in Rehabilitation Services.

Experience Required 1 year: 1 year: Minimum of one year of direct care patient experience, preferably of a generalized nature.

Possession of the PA Physical Therapy licensure.
