Our physician led medical group enjoys an excellent reputation with patients and the medical community, offering primary care, urgent care and more than 30 different specialties.
Job Description
The Population Health Specialist works to ensure patients are up to date with their diabetic and routine health screenings. This per diem position will support Hartford HealthCare quality and balanced scorecard initiatives by working to ensure patient gap in care completion, creating and maintaining necessary training documents, and training offices on workflows to close gaps in care. Performs routine audits to identify training and education opportunities for offices where workflows are not being followed. This is a remote position, however, and the ability to travel to Wethersfield for training is required.
Qualifications
High school diploma or equivalent preferred
Diploma from an accredited Medical Assistant Program and Medical Assistant Certification preferred. 1-2 years Medical Assistant experience in office setting.
Epic experience preferred
