Job Description Job Description North Suffolk is looking for a Psychiatric RN to join our team! This position is central to the Psychiatric and Medical Care Integration (PMCI) model being instituted by North Suffolk Community Services (NSCS) as part of the Community Mental Health Center (CMHC) Project at the Freedom Trail Clinic (FTC).
What You’ll Do:
Provide a broad range of clinical care coordination services for patients with Serious Mental Illness (SMI), homeless or Deaf and Hard of Hearing with major focus on PMCI and psychopharmacology.
Act as the liaison between the patient, the psychiatric & clinical team, the patient’s primary care provider, the pharmacy and other medical / service providers throughout the project.
Support the team with the patient’s enrollment into the CMHC project including obtaining informed consent, initial enrollment measurements, taking vitals and entering laboratory test orders, discussing results with ordering provider, coordinating medical records exchange and follow-up and communication with other medical providers.
Provide patient education about various topics including metabolic syndrome, smoking cessation, routine vaccinations and cancer screening and prevention recommendations.
Have the opportunity to run psychoeducation and support groups with patients who are enrolled in the CMHC grant.
Ensure that state and federal guidelines are followed.
Coordinate between CMHC PMCI psychiatric providers and pharmacy: obtain medication and allergies history, assist with prescription processing and refills, prior authorizations, insurance issues, concerns of diversion, misuse, safety, storage, and behavioral health referrals.
Provide LAI med administration and support psychiatric providers in the clozapine clinic.
What You’ll Need:
Education: Registered Nurse with current Massachusetts license to practice required
bachelor’s degree in nursing preferred.
Experience: Experience in psychiatric, inpatient or outpatient community mental health setting or community health center (primary care) preferred.
Knowledge of psychopharmacology, including administration of LAI antipsychotics and clozapine preferred.
