Job Description:
Seeking a candidate who has flexibility around their work hours to be part of a dynamic health and wellness team that supports consumers of mental health services in addressing their nutrition, fitness, and wellness goals. This is a community-based position working with children, adults, and families. Meetings will be held at the gym, in participants homes, and at other locations in the Greater Manchester area. Requires a valid driver’s license and auto insurance with liability coverage. Certified Personal Trainer certification preferred at start of position but needs to be obtained within the first 3 months. Bachelor’s degree in social sciences, health, or nutrition required.
Requirements:
Key Responsibilities:
Work with participants to identify individualized health and wellness goals.
Provide education and support around nutrition, exercise, smoking cessation, etc.
Complete paperwork and physical assessments at baseline, and quarterly there after.
Develop effective and safe programming for Participants in the gym, in their homes, and/or in the community.
Manage a caseload of 18-25 participants.
Communicate with case managers, doctors, community organizations, etc.
Requirements:
Bachelor’s degree in psychology, social work, counseling, or education/counseling from an accredited college or university.
Personal Trainer Certification preferred at start but must be obtained within 3 months of start.
Mental Health and/or Personal Trainer experience.
Requires a valid driver’s license and auto insurance with liability coverage and reliable car as you will transport participants.

Ability to maintain confidentiality.
Ability to work independently.

Qualifications:
Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy.
Education:
Bachelor’s degree in psychology, social sciences, health, or nutrition is required.
Job type: Full Time 40 hours per week.
