Job description
The Care Coordinator/Service Coordinator functions as a member of an interdisciplinary team to provide care coordination to a caseload of individuals with Traumatic Brain Injury with multiple medical co morbidities and/or co-occurring substance abuse disorders. Essential functions include:
Advocate for and support the client, engage with community agencies/health care providers and others on his/her behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/ or hospitalizations.
Provide clinical support to the Team by providing consultation, education information around conditions, interventions, resources to maintain focus on outcomes and best practices.
Plan and participate in team meetings at least twice per year and as necessary.
Complete a Revised Service Plan and ensure that all attachments are included (Revised Service Plan, Plan of Protective Oversight, E-Med NY, Individualized Service Reports, team meeting minutes from 6 month and 30-day team meeting, waiver rights and responsibilities Provider Contact list and receipt of Provider Contact List).
Conduct initial and ongoing assessments of clients to document strengths, needs, goals and resources.
Maintain documents, records, and other related reports in an organized, timely and accurate manner as per policy and procedure.
Complete monthly face-to-face outreach, engagement and service planning in the field.
Act as a linkage to community services including medical, behavioral, residential, benefits, entitlement and any other needed services per interdisciplinary care plan.
Responsible for advocating for and with an individual to ensure informed decision making, informed consent, and guardianship that is appropriately carried out.
Meet all training requirements on time.
Report abuse or neglect immediately when observed or reported.
Provide crisis intervention and follow-up.
Report to your direct supervisor on-going problems and suggest solutions to improve environment/working conditions
May be assigned other tasks and duties reasonably related to the job responsibilities.
Qualifications
· Bachelor's Degree and two years' relevant experience,
o OR, a License as a Registered Nurse with two years of experience or relevant experience, which can include any employment experience and is not limited to case management/service coordination duties,
o OR, a Master's Degree with one (1) year of relevant experience.
· Strong communication skills, including verbal and written communication skills, along with strong interpersonal and organizational skills also required.
· Excellent organizational, interpersonal, and verbal and written communication skills required.
Job Types: Part-time, Contract
Pay: $24.00 - $30.00 per hour
