Job Description Job Description Qualifications:

Pay range $19-22 per hour.

Requires in person client visits.

Experience:

· Two years of direct care management experience including working independently in the field, conducting biopsychosocial assessments, and developing care plans preferred

· Experience navigating health and social service systems

· Experience linking people with complex health, behavioral health, and social needs to community resources

· Demonstrated ability to support, coach, and motivate participants to reach their goals

· Valid California driver’s license, reliable transportation and proof of current liability insurance required

· Ability to work effectively from office or home daily (private workspace, stable internet connection, etc.)

· Ability to manage time effectively in order to complete tasks and meet deadlines

· Experience and skills using electronic technology (laptop, tablet, cell phone) and Microsoft Office Suite (Excel, Word, and Outlook) required

Education/License/Certification Requirements:

Bachelor or Associate degree in Social Work, Public Health or related field preferred; or licensed LVN or certified CNA/MA; 2-4 years of experience working with underserved and diverse populations in community settings accepted in lieu of degree.

Responsibilities:

Participant Eligibility and Assessment:

· Confirm participant’s eligibility by verifying qualifying chronic condition and acuity criteria with participant.

· Conduct an in-depth assessment with participant including identifying participant’s referring chronic condition(s), Primary Health Concern (PHC), housing stability, SDoH needs, preferences, and barriers on a biannual basis.

· Create a Care Plan with each participant utilizing a SMART goal format and regularly update Care Plan according to participant’s ongoing needs and service utilization to encourage participant’s active participation in better managing their health.

Participant Focused:

· Assess participant’s motivation to change using Readiness to Change scale and use interventions appropriate to participant’s Readiness to Change.

· Utilize motivational interviewing to engage participant and support participant’s motivation to better manage their chronic conditions.

· Monitor ongoing referrals to community resources and services to support participant’s access to health, behavioral health, and SDoH resources.

· Communicate with participant’s health plan and service providers to ensure coordination of services and to advocate for participant’s needs and preferences.

· Help participant to move along the continuum to self-sufficiency.

Participant Education:

· Provide support and education to participants to assist them in adhering to medical recommendations and taking medications as prescribed.

· Provide system utilization education.

· Assist participants in accessing and understanding necessary health and wellness information provided by their medical provider and clinic.
