Home care, Home Health or Hospice experience, or geriatric care education background is beneficial, along with a proven ability to manage a fast-paced, independent work schedule.
The primary focus is to act as the liaison between the agency and the community regarding the client, community, and customers' needs.
Responsibilities include development and promotion of homecare, home health, and hospice programs and strengthening client relationships, while maintaining the standards of practice consistent with quality health care.
