Essential Functions of the position are detailed below, and include any physical requirements below that.
*Tasks include case finding, intakes, comprehensive assessments, screenings, development of individualized plan of care, home visits, prenatal and postnatal Health Education, referrals, transportation assistance with client MCO's and case closures.
*Refer client to their primary care physician for education and services around Reproductive Life Planning, if appropriate.
*Plan and transfer of cases to other case management agencies or home visiting programs after 6 months. All "high risk" infants will be transferred to agencies providing services to the high risk infants.
*Conducts a minimum of 2 home visits. One during pregnancy and one before the child turns 6 months per client, more if indicated by the risk assessment. Conducts a minimum of 1 face-to-face contact per month with each client. Conducts a weekly phone check-in.
*Provide linkage and assistance in accessing postpartum care.
