Nurse/ Care Navigator

We are seeking an experienced, licensed Nurse to help support our multidisciplinary team. This role is part time, Per Diem, based on the Bridgeport/ Fairfield line working with a group of 15-19 older women who are part of a religious community. Most residents are retired with varying levels of care needs. They live in individual units within a large apartment building and gather as desired in a common space for meals, socializing, and scheduled meetings and activities.
Essential Functions:
• Facilitates the implementation of resident's individualized life plan.
• Determine individualized strategy and support needs which may include ongoing active support.
• Advocates for residents needs during medical appointments and clinical settings, as necessary.
• Arranges transportation for and accompanies residents to medical appointments and other meetings, outings, or appointments, as needed.
• May participate in after hours on-call rotation.
• Acts as part of a multidisciplinary team of professionals supporting residents.
• Meets regularly with Team.
Requirements
Education
Bachelor's degree or higher in nursing. Current license required.
Experience:
• Must have a minimum of 5 to10 years of experience in nursing. Experience with older adults is welcomed.
• Knowledge and understanding of social, health, and economic factors that contribute to health and wellbeing outcomes; knowledge and skill in interviewing and assessment (social and health) techniques.
• Knowledge of community resources available to individuals; an ability to mobilize resources into a coordinated and comprehensive plan of care.
• Strong written, verbal and computer skills required.
• Ability to maintain confidentiality of resident, company, and staff information.

Salary: Starting at $45 an hour.
