What You'll Do: We're seeking a dedicated Lead Care Manager to join our Enhanced Care Management Program under the guidance of the Assistant Director. In this role, you will provide direct services, coordinate care management, and function as a vital member of our "Care Team." As the LCM, you will oversee specific cases, coordinating health care benefits, offering education, and facilitating timely and cost-effective access to care. Collaborating with clients, caregivers, and other providers, you will promote wellness, recovery, independence, resilience, and empowerment while maximizing client benefit. As an advocate, you will actively engage with interdisciplinary teams, liaise with external health and social service providers, and contribute to the overall success of our program. Hours:
Monday - Thursday 10am – 7pm, Flex Friday 10am - 6pm

Qualifications
Requirements to Apply:
At least 3 years of relevant work experience
High school graduate or GED required
Experience with usage of EMR/EHRs/Care Management software
Prior experience as a Community Health Worker, Peer Support Specialist, Medical Assistant or similar role a plus
Ability to do home visits and outreach
Interpersonal savvy, demonstrated by the ability to interact with and influence people to establish trust and build strong relationships
Long-time resident of the community with good knowledge of the resources of this community
Requires possession of a reliable automobile and a valid California Driver’s License, as well as a driving record acceptable to the agency’s insurance carrier.
Ability to receive and implement constructive feedback.
