$5,000 Sign-On Bonus for LCSW/LPC (Prorated Based On Weekly Schedule)

The Colorado Coalition for the Homeless seeks an Integrated Health – Behavioral Health Clinician II, Social Worker to provide care that includes assessments, diagnostic evaluations, patient education, crisis intervention and safety/risk assessments, brief and long-term therapeutic interventions, group therapy, care coordination and case management, for individuals with mental/behavioral health and substance use issues. This position will provide individualized treatment, enhanced coordination of care, and case management across the treatment spectrum with goals of maximizing functioning within the community and decreasing need for emergent care. This position is located at 2130 Stout St, Denver, Colorado 80205 and reports to the Associate Director of Integrated Behavioral Health Services.

LCSW/LPC - $70,176 - $80,702 | Salary - Fully Licensed candidates are eligible for a $5,000 sign-on bonus

LSW/LPCC - $29.77 - $34.23 | Hourly

Qualified candidates are eligible for the National Health Service Corps (NHSC) Loan Repayment Program . Licensed health care providers may earn funds towards student loans in exchange for a two-year commitment at an NHSC-approved site through this program.

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing andprivilegingtimeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

Conducts screenings and psychological assessments (including diagnostic evaluations), develops treatment plans and provides MH and SUD therapy services to address the psychosocial needs of clients. Uses a wide range of treatment modalities including motivational interviewing, stepped care, solution-focused therapy, or skills training to address the unique needs of clients experiencing and/or at risk of homelessness.

Provides individualized and enhanced care coordination across the treatment spectrum with goals of maximizing functioning within the community and decreasing need for emergent care.

Provides MH and SUD treatment services to clients and families in face-to-face in-person visits, as well as telephonic and televideo sessions, with an emphasis on care coordination, case management, and assisting clients to access crisis mental health care at hospitals and specialty care facilities, as necessary.

Assists CCH colleagues in understanding significant social and emotional factors related to the clients’ Biopsychosocial problems by providing consultation, education, and advocacy as appropriate.

In conjunction with other CCH Staff, provides accompaniment for specialty appointments to improve patient access to care, as needed.

In conjunction with the CCH Safety Team, supports Milieu Management of the Stout Street Health Center Lobby, and responds to all patient crises and behavioral disturbances in CCH clinical settings, Provides de-escalation and evaluation for M1 Holds and assesses safety to engage with the rest of the SSHC clinic teams.

Leads or co-leads client support groups, educational visits and groups, and substance use and mental health treatment therapy groups.

This job function requires availability and proactive communication during regular business hours.

Assists clients in developing awareness of the need for treatment services, addressing barriers to receiving, and engaging in services. Engagement work includes referral to other CCH services, outreach attempts both on the streets and in various CCH service delivery locations.

Provides clinical supervision and mentoring to MSW, BSW, or other interns as appropriate by licensure level. Passion as an educator for the social workers of the future preferred.

Utilizes the EHR as appropriate, and follows Coalition and BHO/DBH guidelines regarding documentation of services provided.

Maintains on-going competency in the use of the EHR and HMIS which includes:

Recording all patient visits, completing charting, and submitting all billing codes for clients within a 24 hour period;

Maintaining up to date records in ELF on all current and active clinical clients.

Recording telephone and other clinical communications as needed in the EHR.

Managing tasks, PAQ and scanning as needed to have complete and current charts.

Maintaining accurate information regarding the client's insurance status and complete Medicaid authorizations, CCARs DACODES, and other clinical authorization documentation as needed to sustain funding and provide services to individuals served in the clinic.

Utilizing the EHR and any future upgrades as directed.

Maintaining a passing score (above 75%) in all QA compliance audits.

Participates in peer review and peer clinical audits as requested.

Meets formally with SSHC Associate Director of Integrated Behavioral Health regularly and for ongoing supervision.

Attends meetings on time and participates in clinical and administrative meetings as deemed appropriate and necessary for the job.

Works collaboratively as part of a multi-disciplinary team, completing functions necessary to this role.

Demonstrates respect, cultural competence and knowledge of trauma informed care in interactions with patients, Coalition staff, and other community providers.

Performs additional duties deemed necessary and appropriate by SSHC Associate Director of Integrated Behavioral Health. Performs other duties as assigned.

Qualifications Summary

Master’s degree in Social Work, Counseling, or Psychology degree required.

Licensure as an LCSW or LPC required for Level II. (Licensure as an LCSW preferred, as well as certification as a CACII/III or LAC preferred. If provisionally licensed as an LSW the employee will be working actively towards completion of license requirements. If not provisionally licensed at the LSW level, must have been an intern with SSHC to be eligible for hire.)

Experience and interest working with disenfranchised persons and/or people with Severe and Persistent Mental Illness preferred.
