Responsibilities: The Clinical Coordinator LVN is responsible for scheduling, overseeing, and coordinating clinical services provided to patients receiving home health care.The Clinical Coordinator will work closely with the Clinical Branch Manager/Administrator, clinical team, including registered nurses, therapists, and aides, to ensure that patient care is delivered effectively, efficiently, and in compliance with all applicable regulations and standards.Coordinate the scheduling of clinical services, including nursing visits, therapy sessions, and other treatments.Ensure that services are delivered according to the care plan and in a timely manner.Maintain accurate and complete patient records, including assessments, care plans, progress notes, and any changes in patients' conditions.Ensure that all documentation meets regulatory and agency standards.Ensure that all clinical services and documentation adhere to state, federal, and agency regulations, as well as industry standards.Stay updated on changes in regulations and implement necessary changes.Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop and implement a holistic approach to patient care.Maintain open and effective communication with patients, their families, and healthcare providers agency management and staff.Address any concerns or questions and act as a liaison between all parties involved.Other duties as assigned.Requirements Current LVN license in the state of practice.Previous experience in Home Health care is preferred.Strong clinical assessment and care planning skills.Excellent communication and interpersonal skills.Knowledge of healthcare regulations and compliance requirements.Basic computer skills for documentation and record-keeping.CPR certification.Why Should You Apply?
