Our organization is seeking an experienced, dedicated, and compassionate Consulting Case Manager in the Medical industry. The ideal candidate will have a deep understanding of case management, behavioral and mental health. The successful candidate will be responsible for managing patient cases, coordinating with healthcare professionals, and ensuring the best possible care and outcomes for our patients. This role requires a professional who can navigate the complexities of the healthcare system, manage patient care, and make critical decisions under pressure. Responsibilities:
1. Manage patient cases, coordinating with healthcare professionals to ensure the best possible care and outcomes.
2. Provide consultation and advice to healthcare professionals on case management, behavioral and mental health.
3. Develop and implement patient care plans, coordinating with healthcare providers to ensure effective and efficient care.
4. Evaluate patient progress and adjust care plans as necessary.
5. Advocate for patient needs and rights, working to ensure that patients receive the care and resources they require.
6. Collaborate with multidisciplinary teams to improve patient outcomes and healthcare delivery.
7. Maintain up-to-date knowledge of healthcare regulations, practices, and trends.
8. Provide education and support to patients and families, helping them to understand and navigate the healthcare system.
9. Document and report on patient progress, ensuring accurate and timely record-keeping.
10. Respond to critical situations with calmness and professionalism, making decisions in the best interest of the patient.
Qualifications:
1. Bachelor's degree in Nursing, Social Work, preferred but not required.
2. Minimum of 1-3 years’ experience in Case Management in a health care environment
3. Certified Case Manager (CCM) or equivalent certification preferred.
4. Strong knowledge of healthcare systems, regulations, and best practices.
5. Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
6. Strong decision-making and problem-solving skills, with the ability to navigate complex situations and make decisions in the best interest of the patient.
7. Compassionate and empathetic approach, with a strong dedication to improving patient outcomes.
8. Strong organizational skills, with the ability to manage multiple cases and priorities effectively.
9. Proficient in electronic medical records (EMR) systems and Microsoft Office Suite.
10. Ability to maintain confidentiality and adhere to ethical standards.
11. Must be able to work independently and as part of a team.
