Description: This position provides administrative and client support to enhance the business operations of the company and has responsibility for coordinating care for clients which includes serving as an advocate for clients' wants and needs to ensure they receive the best care possible. This position involves extensive telephone work and interaction with clients, clients' families, caregivers and health care professionals. The Client Care Coordinator will also play a supportive role in staffing/scheduling efforts by ensuring reliable coverage by providing direct care for "hard to staff" clients, as well as training and providing quality of care support to caregivers.
Experience/Qualifications: CNA/HHA preferred, experience in the field of home care or eldercare a plus! Minimum requirements include at least 1 year of caregiving experience. Supervisory/office experience is strongly preferred. Must be detail oriented, have excellent customer service skills, be able to manage multiple tasks simultaneously, and be flexible and willing to adapt to unexpected situations. A true team player!
Requirements: Reliable transportation, smart phone, and willingness to drive to/from multiple clients within our service area.
Reports to: Director of Client Care (DCC)
Primary Duties:
CNA/HHA/Direct Care (25%): Acts in the capacity as a CNA/HHA for clients as needed.
Participates in on-call rotation one weekend per month and one weekday per week, or as needed.
Make sure that all cases are covered and arrange for substitute/back-up coverage as necessary.
Assisting with "first shift" introductions of caregivers when attending a shift for the first time with a client.
Client Care Support/Staffing (50%): Prepares for and attends weekly meetings with DCC to review proposed assignments and participate in key client risks and needs.
Provides visibility to staffing and other care challenges, highlights key needs, retention risks and escalates quality of care concerns to the appropriate team members.
Owns responsibilities for assigned clients: delighting clients by ensuring their priorities are being met - including highest reliability, consistency and quality of care with effective communication to client and responsible parties.
Responsible for maintaining client files/profiles and keeping them up to date, and promptly informing team members in the office about changes.
Works collaboratively with the DCC to on-board new clients.
Works closely with the Office Manager to ensure all assigned clients are appropriately and adequately staffed with great matches.
Caregiver Support (25%): Assist with caregiver growth and retention.
Provide support to caregivers in the field to ensure they are properly equipped to provide the care needed for their client(s).
Dropping off needed supplies to caregivers as needed to provide care for their clients (ex. masks, gloves, and other PPE as needed).
Provides support for other team members as needed: Tech/System Support: Willingness to learn and understand software systems/tools (AxisCare, Streams phone system), and apply this knowledge when needed.
Represent Assisting Hands outside of the office with the intent to help grow the business and increase awareness of our services offered.
Hours: 24 hours/week over 4 days/week, 8am-2pm. Participates in on-call rotation; weekly scheduled hours are flexible with approval of DCC. May include some evening/weekend hours as needed.
FLSA Status: Hourly, non-exempt
Compensation: $20.00 - $24.00 per hour
