This role develops, implements, manages, and evaluates customized, evidence-based, innovative digital and multi-model patient/family education and support tools, projects, and programs that educate, engage and support patients and families. Integrates Supportive Care into tools, projects, and programs in order to expand awareness of supportive care and market programs and resources. Drives integration of resources and tools into patient care.

Recommends, monitors, and reports metrics to assess the impact of digital and multi-modal patient/family education. Analyzes and improves patient and family education processes. and leads and participates in interdisciplinary process improvement initiatives.

Works closely with Supportive Care program managers and director to build relationships across the department and the system in order to advance the Department of Supportive Care Medicine's strategic priorities. Collaborates with Nursing, providers, other clinicians, Patient Communications, Digital Marketing, Digital Innovation, Patient Access and other City of Hope departments as well as with patient and family advisors.

As a successful candidate, you will:

* Develop, implement, coordinate, and evaluate customized, evidence-based, digital and multi-modal patient and family education / Supportive Care Medicine tools and programs across multiple platforms (e.g., website, Epic/MyCityofHope, multi-media, social media, digital screens).

* Provide specialized health education consultation to clinicians, executives regarding development and evaluation of multi-modal patient and family education.

* Coordinate and facilitate engagement with patient and family advisors in digital and multi-modal patient and family education / Supportive Care innovation projects.

* Manage patient and family education projects; develop project plan, project expenses, negotiate cost-sharing, achieve milestones by target dates and meet budget targets.

* Work closely with program managers to build relationships across the department and system in order to enhance patient and family education/engagement and advance the Department of Supportive Care Medicine's strategic priorities. Collaborates with Nursing, providers, other clinicians, Patient Communications, Digital Marketing, Digital Innovation, IT, Patient Access and other City of Hope departments as well as patient and family advisors.

* Project manage development of tools, programs and projects. Develop a project plan with defined milestones, target dates and budget, including cost-sharing. Negotiate cost-sharing at inception of project. Monitor progress toward achieving milestones and target dates; execute changes as needed to achieve targets.

* Analyze patient and family education processes for specific patient and caregiver populations and for City of Hope patients and caregivers as a whole. Lead and participate in interdisciplinary process improvement teams.

* Recommend, monitor and report metrics to assess the impact of patient and family education tools and programs on quality and financial outcomes. Continually refine tools, programs and processes based on evaluation and metrics.

* Provide essential health education expertise for other strategic Supportive Care Medicine projects and initiatives.

Your qualifications should include:

* Master's Degree in Public Health or related field with substantial coursework in health education.

* 2 - 3 years of professional health education experience.

* Program development and evaluation experience.

* Use of innovative technologies within health education programs.

* Certified Health Education Specialist (C.H.E.S.).

* Preferably: Major in Health Education. Master Certified Health Education Specialist (M.C.H.E.S.). Health education experience in a hospital setting, including experience in digital communications and project management.
