Nurse Care Manager

Overview:
As a Nurse Care Manager at Anna Bissonnette House, Elsie Frank House, Burroughs Street, Ruth Cowin House, and Olmsted Green, your primary responsibility is to ensure the health and well-being of our residents/clients. You will play a crucial role in advocating for their needs, providing timely and appropriate access to care, and coordinating with various healthcare professionals to optimize their overall health outcomes.
Major Functions:
• Advocating for residents’ mental and physical wellness.
• Educating residents about their current health status and providing timely information and care.
• Investigating and enrolling residents into SCO's (Senior Care Options) to ensure comprehensive care.
• Coordinating with doctors' offices, visiting nurses, home care agencies, pharmacies, PCPs (Primary Care Physicians), therapists, psychiatrists, and medical specialists as needed.
• Scheduling annual doctor visits for residents.
• Document any changes in residents' health status accurately and promptly and update internal database.
Duties and Responsibilities:
Assessment:
- Conduct comprehensive assessments of residents' health status upon admission into the supportive housing facility, follow up as needed.
- Communicate findings and provide relevant information to residents in a timely manner.
- Develop and update care plans in collaboration with the interdisciplinary team.
- Document any changes in residents' health status accurately and promptly.
Education/Advocacy:
- Educate residents about their current health status, medications, and wellness practices.
- Advocate for residents' needs and ensure they receive appropriate care and support. Maintain CDC suggested vaccinations in accordance with the EOEA.
- Investigate and enroll eligible residents into Senior Care Options (SCO) for enhanced care coordination.
Coordination:
- Establish and maintain effective communication channels with healthcare professionals and service providers.
- Coordinate appointments and services with doctors' offices, visiting nurses, home care agencies, pharmacies, PCPs, therapists, psychiatrists, and medical specialists.
- Schedule annual doctor visits for residents and ensure follow-up care as needed.
Supervision of Personal Care Home Makers:
- Manage schedules, time-off requests, and performance evaluations for Personal Care Home Makers.
- Conduct biweekly meetings with Personal Care Home Makers to discuss client needs, updates, and any concerns.
- Perform annual evaluations and performance reviews for Personal Care Home Makers, and manage and monitor appropriate day-to-day interactions, identifying areas for improvement such as attitudes, professionalism, and timeliness.
Requirements:
- Registered Nurse (RN) or Licensed Practical Nurse with a degree from an accredited School of Nursing (B.S. preferred).
- Minimum of two (2) years clinical experience.
- Experience in PACE, GAFC, Dementia, Elders, Persons with Disabilities, or Community Health Services preferred.
- Minimum of one (1) year supervisory experience preferred.
- Must maintain nursing license in good standing.
- Must maintain CPR certification.
