Description National Accounts Coordinator

As a key liaison between Lakeview Health, other facilities, and professionals representing the next stage in the continuum of care. Join us in our mission to empower individuals on their journey to recovery and wellness.
As a National Accounts Coordinator with Lakeview Health, you will work with the treatment team to assist the patient in their integration into the program; coordinating with outside resources to ensure the patient can focus on their recovery. You will also work with the treatment team and the patient in executing and facilitating post-discharge treatment arrangements with outpatient clinics, private therapists, extended residential facilities, half-way houses, etc.

Finding the perfect match

Bachelor’s degree in Social Services, Psychology, Business, or related field.
One year case management experience, preferred.
One year experience working in the field of addiction, preferable with the adult dual diagnosis population.
Basic computer knowledge and familiarity with Microsoft Office (Word, Excel, Outlook).
Schedule:
Monday-Friday 8AM05PM, Some nights and weekends.
